Effects of hormone replacement therapy on the breast

Abstract

Aim: To observe the effect of hormone replacement therapy (HRT) on the breast, as seen on mamniography, and to determine the inter-observer agreement in classification of breast parenchymal pattern. Methodology: This is a retrospective study of 45 women on HRT. Two radiologists studied the pre-HRT and post-HRT mammograms for: (i) changes in breast parenchymal pattern, (ii) fibroadenoma or cyst formation, and (iii) malignant change. The breast parenchymal pattern was classified according to Tabar. The inter-observer agreement in the classification of parenchymal pattern was determined. Result: The majority of women did not show any change in parenchymal pattern. Only four women showed a change in pattern and only one of them showed an increase in density. These findings were not statistically significant. There was no fibroadenoma or cyst formation and no malignant change. The Kappa value of 0.240 for inter-observer agreement reflected poor agreement between the two radiologists. Conclusion: HRT was not shown to cause any significant change in the breast. There was a poor inter-observer agreement in classification of breast parenchymal pattern.
